Here’s a clear checklist for completing all required paperwork and links before your child can begin attending. Please read carefully and follow the steps according to your site location.

✅ STEP 2: Choose and Complete ONE Packet Based on Your Location

⛪ A. Church Site Packet

For families attending:
Apple Valley (Grace Lutheran), Forest Lake Area (Faith, Hosanna, and St. Paul), Golden Valley (Valley of Peace), White Bear Lake Area (Community of Grace and Redeemer), Stillwater Area (Our Savior’s and Trinity), Minneapolis (Trinity Falls)

Complete all of the following:

• Child & Family Information / Permissions

• Emergency Form / Authorized Persons

• Health Care Summary (Signed by clinic/physician; 30-day grace period)

• Immunization Form (Parent-completed or clinic printout)

• Toddler Individual Daily Schedule (If child is a toddler, 16-32 months old)

• Marketing Release or Opt-Out Form
• Tuition Express Payment Form

• Additional INFANT FORMS (if child is an infant, 6 weeks-15 months old):

  • Swaddling Consent Form

  • Physician’s Directive for Alternative Infant Sleep Positions (Signed by physician)

  • Rolling Over Parent Statement

  • Infant Individual Daily Feeding Schedule

  • Infant—Wear Helmet While Sleeping (Signed by physician)

🏫 B. Catholic School Site Packet

For families attending:
Eagan (Faithful Shepherd), Mahtomedi (St. Jude of the Lake), Maplewood (Presentation of Mary), Minneapolis (Annunciation, St. Helena), Minnetonka (Notre Dame Academy), Savage (St. John the Baptist), Wayzata (St. Bart’s)

Complete all of the following:

• Child & Family Information / Permissions

⚠️ REMINDERS:

• All forms must be submitted BEFORE your child’s first day, except the Health Care Summary, which has a 30-day window.

• Double-check if your site is a Catholic School or Church site to ensure you complete the correct packet.
